Guidelines to Receive Home Health Care

In order to receive home health services, the patient must be homebound.

A homebound patient is defined as one who requires considerable effort and assistance to leave home. Medicare guidelines require that the care given must be medically reasonable, necessary and ordered by a physician. The patient must require skilled care that can be administered intermittently.

**Contact your primary care physician to discuss home care for your needs. Your physician will contact Phoebe Home Care to start the admission process.

What must I do if I have a need for home health care?

Contact your physician or social services department of your local hospital. Ask them about Phoebe Home Care
